A new Weizmann Institute study advances our understanding of the genetic code: It proposes a way of effectively introducing numerous carefully planned DNA segments into genomes of living cells and of testing the effects of these changes. The study is being reported in the June issues of Nature Biotechnology and Nature Genetics.

Until now, changing the DNA sequence has been a slow and labor-intensive process. It took several weeks to alter just one DNA region at a time; testing the effects of each of these changes took even longer. In the new study, Weizmann Institute scientists have developed a technology that makes it possible to simultaneously introduce tens of thousands of DNA regions into tens of thousands of living cells ? each region in a separate cell ? in a planned and systematic manner, and to measure the results of each such change with great precision and within a single experiment.

"This fast method will significantly advance our ability to understand the 'language' of DNA," says research team leader Prof. Eran Segal, of the Weizmann Institute's Computer Science and Applied Mathematics and Molecular Cell Biology Departments. "Reading out a person's entire genome is already a manageable task, but what exactly is written in that genome? After all, a genome looks like a lengthy string of letters whose meaning is for the most part obscure. Just deciphering the DNA letters is like trying to understand a foreign language by listening to it being spoken. Our method will help us identify DNA 'words' and understand their meaning."

Understanding what's written in the DNA might help us interpret, among other things, how genotypic differences among people generate observable differences among them, from the way we look to the way our cells function. Thus, for example, it might be possible to clarify which genetic differences are responsible for the development of various diseases in certain individuals. The Weizmann Institute technology can also lead to improved genetic therapies based on introducing new genes or improved regulatory sequences into cells in order to repair genetic defects.

In the present study, the scientists investigated a vital aspect of the DNA language: How the control of gene expression is encoded in the DNA ? that is, the instructions determining the level of activity of each gene in the genetic code. Since gene activity levels have crucial effects on cell function, this question, considered one of the central in molecular biology, has been studied for decades. The new technology has enabled the scientists to isolate and test the effects of various parameters on gene activity levels: For example, how a gene's activity level is affected by the gene's distance from its regulatory sequence. The researchers have managed to elucidate how various parameters define the regulatory "language" and to demonstrate how deliberate changes in the genetic sequence affect these parameters in a way that alters the level of a gene's activity in a predictable manner.

The new method consists of four steps that combine existing technologies in an innovative manner. The steps are: creation of 50,000 different genetic sequences on DNA chips; massive insertion of these sequences into cells at the same time; sorting the cells with the help of a sorting machine that senses the expression levels of a "reporter" gene; and high-throughput parallel DNA sequencing.

A recent analysis of data from the SEER population-based database estimates that the 5-year survival rate for Stage IV patients diagnosed between 1991 and 2000 was 8%.2 However, despite a low overall cure rate, aggressive treatment is indicated for most patients to extend survival and enhance quality of life. Systemic chemotherapy, endoscopic treatments to palliate obstruction, surgical diversion, and surgical resection all have important roles in treatment of Stage IV patients. Treatment approaches must be individualized based on the extent and resectability of local and distant disease, the presence or absence of bowel obstruction, performance status, and comorbidities. For patients with good performance status and minimal symptoms from their primary cancers, standard treatment is systemic chemotherapy, which is well documented to increase survival and quality of life.3,4 Surgical resection of the primary tumor and, when feasible, of the metastatic lesions can provide excellent palliation and can, in some cases, provide lasting cure.

Dana-Farber: Study reports first success of targeted therapy in type of non-small cell lung cancer

BOSTONA novel compound has become the first targeted therapy to benefit patients with the most common genetic subtype of lung cancer, an international clinical trial led by scientists at Dana-Farber Cancer Institute and other institutions will report at the annual meeting of the American Society of Clinical Oncology (ASCO) June 1-5 in Chicago.

Pasi A. Jnne, MD, PhD, scientific co-director of Dana-Farber's Belfer Institute for Applied Cancer Science, will present the findings from the randomized phase II study (abstract 7503) on Monday, June 4, 3 p.m. CT, E Hall D2, McCormick Place.

The study involved 87 non-small cell lung cancer (NSCLC) patients whose tumors carry a mutation in the gene KRAS. Such tumors account for about 20 percent of NSCLC cases, but no targeted therapy has proved effective against them in previous clinical research. The drug under investigation, selumetinib, doesn't attack KRAS directly, but interferes with one of its molecular henchmen, a protein called MEK.

Participants in the study all had advanced stages of the disease. They received the standard chemotherapy agent docetaxel in combination with either selumetinib or a placebo.

By many measures the rate and duration of response to treatment, change in tumor size, and proportion of patients alive and showing no signs of advancing disease -- the group receiving selumetinib did significantly better than the other group. Most clinically significant were the improved rate of response to treatment (37 percent compared to 0 percent in the placebo arm) and prolonged progression-free survival (5.3 months compared to 2.1 months in the placebo arm). Although patients in the selumetinib group survived longer, on average, than those in the placebo group 9.4 months compared to 5.2 months the improvement was not considered statistically significant.

"This clinical trial demonstrates that a combination of chemotherapy and selumetinib is significantly better than chemotherapy alone for this group of patients better in terms of tumor response to therapy and in terms of survival times prior to advance of the disease," says Jnne. "It suggests that for the first time we may have an effective treatment for KRAS-mutant lung cancer, which is the largest single subtype of the disease. These impressive clinical findings not only have implications for the treatment of lung cancer but all cancers that harbor KRAS mutations, including pancreatic and colorectal cancer."

Some side effects, such as neutropenia (a white blood cell deficiency), loss of strength, acne, and respiratory problems were more common in the selumetinib group than the other, but the rate of patients dropping out of the study because of severe side effects was similar for both groups.
